Questionnaire Format
Core Structure
Each survey contains a minimum of 12 required quantitative questions using a standard rating-scale format (e.g., “Strongly Agree” to “Strongly Disagree”) for consistency and comparability across courses. There are 12 categories:
- 4 questions on Instructional Design
- 4 questions on Instructional Delivery
- 4 questions on Assessment
Every survey also includes 2 mandatory open-ended (qualitative) questions, where students can provide written comments.
Faculty may add up to 2 additional custom open-ended questions to collect more specific feedback.
Minimum total questions: 14
Maximum total questions: 24
Optional custom text questions: up to 2
Customization Options
Instructors can select which quantitative questions appear on their surveys for each of the 12 categories. At least one question must be selected for each category. Up to 2 additional custom open-ended questions can be added to collect more specific feedback.
Instructors who take no action will automatically receive the default set of questions (1 from each required category) along with the two mandatory open-ended (qualitative questions.)
